New Patient Instructions

• Remember to bring your health insurance card and copay
• Make sure you obtain a referral prior to your visit, if necessary
• Here are directions to our location
• Please come 15-20 min before your appointment to fill out this medical history form

What to expect:

• After arrival, you will be asked to sign  saying that you read our Financial Policy and HIPAA privacy notice policy
• Your appointment is scheduled for  30 minutes. We typically run on time
• In consideration of our other patients, if you are late for your appointment, or require additional time we are happy to schedule a follow up appointment to provide you the time you need
• Most patients are first seen by one of our PAs, Anna or Lauren, who are expert hernia physician assistants. Dr. Fullington will then come in for an examination to confirm the diagnosis and plan.
